Media Release and Authorization

I hereby authorize Great Bend Center for Music and those acting on its behalf to photograph, videotape, audio record, or otherwise record the likeness, voice, performance, and/or creative work of myself or my child(ren) in connection with rehearsals, performances, classes, workshops, community events, and other activities of Great Bend Center for Music.

I understand that these photographs, video recordings, and audio recordings may be used to document and promote Great Bend Center for Music, its programs, performances, activities, and mission. I understand that recordings may be edited for length, format, sound, image quality, or presentation, but will not be digitally manipulated in a way that materially misrepresents me, my child(ren), or the circumstances in which the recording was made.

I grant Great Bend Center for Music permission to use, reproduce, publish, distribute, display, and share these photographs, video recordings, and audio recordings, in whole or in part, for promotional, educational, archival, fundraising, and community-engagement purposes. Such uses may include, but are not limited to, websites, social media, email communications, advertisements, printed materials, press and media materials, program and event promotions, grant and funder communications, and other digital or print publications.

I waive the right to inspect or approve the finished photographs, recordings, publications, or other materials in which my likeness, voice, performance, or that of my child(ren) may appear.

I release Great Bend Center for Music and those acting on its behalf from claims arising from the authorized use of these materials, including claims related to privacy, publicity, or other personal or proprietary rights. I understand that recordings created by or on behalf of Great Bend Center for Music may be retained by the organization for archival and future authorized use.

I understand that neither I nor my child(ren) will receive compensation for participation in these photographs, video recordings, or audio recordings, or for their present or future authorized use by Great Bend Center for Music.

I have read and understand the terms of this Media Release and Authorization and voluntarily agree to them.
